Criminal history background check/Police Clearance Certificate is issued by the Wisconsin Department of Justice Crime Information Bureau (CIB).This department maintains the central fingerprint based criminal history for Wisconsin, the Centralized Criminal History (CCH) database contains detailed information of arrests, prosecution, findings in court, state correctional system admissions and releases. The database is an accumulation of information submitted by Wisconsin law enforcement agencies. CIB provides. This process is initiated by submitting a Wisconsin Criminal History Challenge Form (DJ-LE-247) and a full set of fingerprints.

Procedure of Getting Police Clearance Certificate

In order to obtain a Police Clearance Certificate from Wisconsin, applicant required to fill an application form which includes name, sex, and race, date of birth, reason fingerprinted and information regarding the official capturing the fingerprints. Then, obtained a set of fingerprint impression (ten prints) which were taken by a qualified fingerprint officer at our agency. The original fingerprint card must be posted with applicant’s completed application form, then the certificate will be posted to the forwarding address as indicated by the applicants in the application.

Documents required for Police Clearance Certificate

  1. A detailed document explaining the purpose for which the Police Clearance Certificate is requested along with a letter stating the period of legal stay in Wisconsin
  2. Four recent colored passport size photographs
  3. Valid document of work permit, dependent pass of the applicant
  4. Fully completed application form which is required to filled in BLOCK LETTERS only
  5. Copy of all of the passport pages (old and current) with copies of the first three of the data pages of his/her passport as well as the pages containing Wisconsin visa entry and departure stamps
  6. Copy of identity proofs (passport, driving license or any similar document)
  7. Wisconsin identity card issued (if any)
  8. A set of applicant’s fingerprint impressions which is required to submit and duly attested by our organization
