check/Police Clearance Certificate is issued by the Wisconsin Department of
Justice Crime Information Bureau (CIB).This department maintains the central
fingerprint based criminal history for Wisconsin.in Wisconsin, the Centralized
Criminal History (CCH) database contains detailed information of arrests,

In order to obtain a
Police Clearance Certificate from Wisconsin, applicant required to fill an
application form which includes name, sex, and race, date of birth, reason
fingerprinted and information regarding the official capturing the
fingerprints. Then, obtained a set of fingerprint impression (ten prints) which
were taken by a qualified fingerprint officer at our agency. The original
fingerprint card must be posted with applicant’s completed application form,
then the certificate will be posted to the forwarding address as indicated by
