In order to obtain a Police Clearance Certificate from Wisconsin, applicant required to fill an application form which includes name, sex, and race, date of birth, reason fingerprinted and information regarding the official capturing the fingerprints. Then, obtained a set of fingerprint impression (ten prints) which were taken by a qualified fingerprint officer at our agency.
